DSPL2 Post 16 Transition Coordinator
PURPOSE OF THE JOB
- To work with the secondary schools within DSPL2 to identify vulnerable young people with SEND who will need additional support to manage the transition to Post 16 education, employment or apprenticeship.
- To provide guidance and support to the identified young people and families, and liaise with the school and Post 16 setting during the transition process.
- To monitor the young people’s progress and attendance in the Post 16 setting, making changes as appropriate, to ensure the young person does not become NEET (Not in Education, Employment or Training).
Supporting young people
- Support the pastoral and educational needs of young people with SEND and without an EHC plan, for whom the Post 16 transition process will prove difficult without extraordinary measures
- Nurture a team approach to developing rapid and direct support as needed
- Support effective communication between various support agencies/mechanisms
- Make information sharing between schools and agencies / support services more efficient and directly meaningful to the needs of the student
- Support emotional well-being for vulnerable young people and their families
- Support the needs of the family in supporting the needs of the young person through transition and the early period of Post 16 education
- Reduce the number of young people who are in danger of becoming NEET.
- Communicate need and impact to the DSPL2 Board termly
- Assist in the formulation of programmes of study for individual young people transferring from secondary to Post 16 settings
- Liaise with teaching staff to support teaching and learning
- Liaise with the DSPL2 Manager / Area SEND Lead
- Attend meetings and case conferences where necessary
- Complete paperwork and administrative tasks relating to the young people, including arranging meetings
- Provide a telephone response system signposting services for local secondary schools
Supporting Schools and Post 16 Settings
- Where appropriate, foster links between home and school.
- Liaise, advise and consult with members of other support teams
- Contribute to reviews of young people’s progress as appropriate
- Participate in relevant professional development.
- Follow school policies and procedures.
